Liability Forms Notice

If you will be attaching your liabilty forms, please combine the signed forms into one pdf and use the paperclip icon at the bottom of the form to attach.

Alternatively, you can follow one of the other options below.
  • Packet and/or Forms can be e-mailed to Grace Faulkner at FaulknerGR@elpasotexas.gov
  • Bring the packet and/or forms in person weekdays from 9:00 a.m. to 4:00 p.m. at the Grandview Senior Center, 3134 Jefferson Ave. OR to any of the mandatory orientation meetings.

Signing

Payment can be made over the phone by calling (915) 566-1217 or in person once the application has been approved.
